Our Appraisal Pathway takes patients through a process of personal, medical and gender-related questionnaires and allows the sharing and gathering of information on which to base your care plan. If you would like to receive care that is subsidised by our GenderGP Fund, please complete the Appraisal Pathway Form which will provide an option to complete a statement of means.
Fill this out as fully as you can and provide any supporting evidence you have in terms of assessments, reports, letters etc that may have already been carried out, or current prescription medication that you are taking. When all the information has been received, we will work with you to make a plan for yours or your loved one’s care.
